Is Eastern always exactly three hours ahead of Pacific?

Almost always. The two coasts change their clocks on the same dates — forward on the second Sunday in March, back on the first Sunday in November — so they travel together: PST (UTC−8) alongside EST (UTC−5) in winter, PDT (UTC−7) alongside EDT (UTC−4) in summer. Three hours either way. The gap breaks only on the changeover mornings themselves, because each zone flips at 2:00 AM local time. In March, Eastern springs forward three hours before Pacific does, so from 11:00 PM Pacific on the Saturday until 2:00 AM Pacific on the Sunday the coasts sit four hours apart rather than three.
